Supported Living & Outreach

Supported Living

At Precedo Healthcare Services, we provide supported living for adults and young people who need personalised, flexible support to live safely and independently in their own homes.

We support people with a wide range of needs. From those with lifelong conditions such as learning disabilities or autism, to individuals with more complex and changing circumstances, including young adults with experience of trauma, homelessness, care, or abuse.

We believe that everyone deserves a stable home, respectful relationships, and the opportunity to shape their own future. That’s why our approach is always person-led, trauma-informed, and grounded in positive risk management.

Who we support

  • Adults with learning disabilities, autism, or complex health needs
  • Young people and care leavers transitioning to adulthood
  • People with mental health conditions, including dual diagnosis
  • Individuals with a history of homelessness or housing instability
  • People with experience of trauma, abuse, or exploitation
  • Those with behaviours that challenge or self-neglect

What we offer

  • Safe, stable accommodation with flexible support built around the person
  • Support with daily living skills, personal care, routines, and self-management
  • Help with tenancy sustainment, budgeting, and community access
  • Medication support and access to healthcare
  • Emotional support, confidence-building, and trauma recovery
  • Positive risk planning and safeguarding in collaboration with partners
  • Support to access education, employment, volunteering or training
  • 24/7 on-site or visiting support based on assessed need

Our experienced teams work in partnership with local authorities, NHS services, housing providers, families and advocates to deliver joined-up, outcomes-focused support. We take pride in creating environments where people feel safe, included, and empowered.

Community Outreach

Our Community Outreach services provide flexible, person-centred support to adults and young people living in their own homes – including those in temporary housing, supported accommodation, or transitioning to independence.

We work with people who may not need 24/7 care but benefit from regular, trusted support to stay safe, well, and connected. This includes individuals with lifelong needs as well as those with more complex life experiences, such as trauma, homelessness, or system involvement.

Our outreach teams take a holistic, trauma-informed approach, recognising that meaningful support is built on relationships, consistency, and understanding each person’s unique journey.

Who we support

  • Adults and young people with learning disabilities or autism
  • People with mental health needs or dual diagnosis
  • Young adults, including care leavers and those at risk of homelessness
  • Individuals with complex risk profiles, including self-neglect or exploitation
  • People recovering from trauma, abuse, or breakdown in family support
  • Anyone who needs help to live independently and reduce risk

What we offer

  • Regular wellbeing check-ins and practical support at home
  • Help with daily tasks like shopping, cooking, attending appointments, and managing medication
  • Support to access housing, benefits, healthcare and other essential services
  • Emotional support and confidence-building
  • Routine-setting and life skill development
  • Community access — including volunteering, learning and social opportunities
  • Multi-agency working to manage complex risk, safeguarding, or care planning

Our aim is always to do with, not for. Helping people build the skills, confidence, and connections to live the life they want. Whether someone needs short-term help after a crisis or long-term support to maintain independence, we work flexibly to deliver what matters most to them.
